ผู้ป่วยและญาติ Patient RelatedPerson

ผู้ให้การรักษาพยาบาล Practitioner

ผู้ให้การรักษาพยาบาล อาจเป็นแพทย์ ทันตแพทย์ เภสัชกร พยาบาล นักเทคนิคการแพทย์ นักกายภาพบำบัด นักโภชนากร นักสังคมสงเคราะห์ ผู้บันทึกข้อมูล เจ้าหน้าที่ห้องบัตร

ข้อมูลรายละเอียดอาจเชื่อมต่อกับฐานข้อมูลบัตรประชาชน Person

การเจ็บป่วย มีการดูแลรักษาต่อเนื่อง เช่น ปวดท้อง ไปตรวจครั้งแรก มีการนัดตรวจเลือด ปัสสาวะ ตรวจเอกซเรย์ เข้านอนโรงพยาบาล ผ่าตัด ดูแลหลังผ่าตัด จำหน่ายออกจากโรงพยาบาล นัดไปตรวจ ทำแผล ให้ยาต่อเนื่อง เรียกว่า 1 Episode

ใน 1 EpisodeOfCare มีหลาย Encounter ซึ่งอาจมีการนัดตรวจ Appointment
