Title: Health Professionals’ Attitudes, Beliefs and Approach to Practice Change about Pediatric Pain in Northeastern Thailand.

Abstract

Background Despite mounting evidence to support pain management practices for children, many do not receive adequate pain care. Challenges have been identified, but primarily within a Western context. This study is part of an action research program being conducted by Thai and Canadian researchers to improve pediatric pain care in Northeastern Thailand. We explored clinician beliefs, knowledge, and practice experience, and captured data on perceived barriers and challenges to improving pain care. The findings will inform the intervention phase of the main project.

Method Health professionals (physicians, nurses, and pharmacists) responsible for pediatric care in 7 hospitals (4 provincial, 2 regional, 1 university hospital) were surveyed using a self-administered questionnaire about seeking knowledge, approach to practice change and a test of knowledge, attitudes, and beliefs about pediatric pain management.

Result Overall response rate was 56.5 % (591/1,049). Most responders were nurses (81%). Pediatric pain management was considered to be important in their unit by 87.1% of responders. They indicated that guidelines or policies are helpful in providing needed care (97.4%). However, less than half of them (35.5%) had time to regularly read journals. Discussion with a senior physician was the most preferred method (79.9%) when there is no research evidence to guide practice. The internet was not the preferred medium (30.7%) compared to the telephone (67.9%) for seeking professional assistance. When deciding on treatment for a child, 93.9% agreed that it is very important to understand parents’ or patients’ choice. The most preferred method to learn and practice new techniques was workshop attendance (33.9%). For knowledge and attitudes about pain management, the mean individual test score was 48.6 % correct.

Conclusion: Pediatric pain is a concern in Northeastern Thailand. Guidelines, policies, and workshops were preferred strategies for improving care.
