Abstracth

Introduction and objective : In 2017 , Thailand has 11 million elderly population,

Encountered chronic diseases and high costs , need long term care focus system District Health Board.

Methodology : This study was Quality research,the simple ware 30 people, who had been offered from October 2017 – September 2018, Use the chip model as an evaluation framework. The data were analyzed for the percentage , Content analysis.

Results : 1) Concerning context, Policy and strategy of ministry of Public Health and health problems. 2) Import factors, Sufficient personnel and budget important knowledge cultural visit to the home of the elderly. 3) Process, There is a matching understanding, under the coordination mechanism at all level. 4)Development results, Participating 6 district long term care , 7 local administrative organizations, Caring for 203 elderly beds, Elderly caregivers who trained 70 hours 178 people, Care manager 15 people, local government organization establish an elderly care center management support compensation to create a mechanism long term care , driven and integrated link to District Health Board, support health promotion, self care, to support the transition to the elderly society, and long-term care in the community.

Discussions : In 2018 , guidelines driving issues for long term care development use the concept manage health problems and community health services, the highlight is the development of public sector role local government use the process PDCA (Demine Cycle)

Conclusion The : District Health Board support network partners to fix the problem long term care based on community context to formulate policies and strategies in integrated health managementfor the elderly to live longer with the elderly society.
