Neuropathic foot
 minimum = รองเท้าแตะมีสายรัดส้น
-รองเท้าหุ้มมิดชิด ขนาดพอดีเท้า

 Intrincsic muscle weak จะทำให้ผู้ป่วยจะไม่สามารถ holdรองเท้าอยู่ ทำให้รองเท้าหลุดบ่อยๆ ผู้ป่วยจะจิกเท้าไว้กับรองเท้า ซึ่งจะทำให้เกิด Claw toe ตามมา การใส่สายรัดส้นเป็นการป้องกัน การจิกเท้าซึ่งทำให้เกิดแรงกดมากบริเวณปลายนิ้ว
-ป้องกันTruamaเพราะผู้ป่วยรับความรู้สึกผิดปกติ
Neuropathic foot + deformity
- Claw toes
- Hammer toes
- Bunion
 Minimium=Simple sandal has back strap with orthotic
 Orthotic:-Arch support
-        metatarsal pad
-        pronator /supinator wedge

เนื่องจาก deformity ทำให้เท้ามีการลงน้ำหนักที่ผิดปกติไป ซึ่งอาจเกิดหนังแข็งและแผลจากpressure sore ในบริเวณนั้นได้การมี Mould Insole or orthotic ที่เหมาะสมจะช่วยลดจุดกด ในบริเวณที่มีแรงกดสูง
Neuropathic foot + deformity + History of ulcer/ marked scarring
 Minimium: Insole or moulded Sandal

เพิ่มบริเวณรับน้ำหนักและลดการลงน้ำหนักบริเวณที่เคยมีแผล เพื่อป้องกันการเกิดซ้ำของแผล
Charcot foot or Unstable ankle
 minimium:custom mould shoe/Boot with moulded insole and rigid rocker undersole
“ตลอดเวลาที่ยืนและเดิน”

เนื่องจากframe workถูกทำลาย custom moulded shoe จะช่วยเพิ่มในเรื่อง stabilityและเข้ารูปกับเท้าที่ผิดปกติมากๆ ส่วน rigid rocker under sole จะช่วยให้ผู้ป่วยที่มีframe work abnormality สามารถเดินได้ง่ายและลดpressureที่fore foot ตอน take off

 

-  แต่ถ้ายังมีแผลอยู่ควรเป็นรองเท้าเพื่อการรักษาแผลเช่น Half shoe ,sandal with metatarsal bar เป็นต้น
-  ตารางนี้จะเป็น Minimum requirement ที่ต้องให้สำหรับผู้ป่วยซึ่งจำเป็นในงานป้องกันการเกิดแผลและยับยั้งการเกิดแผลซ้ำหลังจากแผลหายแล้วแต่เราสามารถให้ Intervention อื่นที่เกินกว่านั้นได้ เช่น
           Neuropathic with claw toe minimum is sandal with back strap
แต่เราสามารถให้ Sport shoe with moulded insole ได้
Intervention ที่เป็น gold standard สามารถให้กับเท้าทุกระดับยกเว้น Charcot foot
-        Sport shoe with moulded Insole
-        Adjustable custom moulded shoe with moulded Insole
-        Custom shoe ( Velcro or lace up) with moulded Insole
