History
Case 1 : Resected 15 cm. mass at segment #4 liver from a 74 year
old male.
Case 2 : Left cervical mass biopsy from 24 year old male
Case 3 : Right pleurectomy from 19 year old female who came with
sudden spontaneous pneumothorax
Case 4 : Right cervical mass from 27 year old male
Case 5 : Slide of cervix from from hysterectomy specimen from a 55
year old female who had report of atypical gland from endometrial
curettage. This case is for differential diagnosis
Case 1 :
Dx : Angiomyolipoma
- caseนี้ tumor ประกอบด้วย tortous blood vv. , smooth m., fat, hematopoietic cell, foam cell
- epithelioid cell มี eosinophilic cytoplasm, prominent nucleolus
- ถ้าย้อม IHC จะติด HMB-45, actin, desmin +ve
Case 2 , Case 4 : เป็นผู้ป่วยรายเดียวกัน อายุ 48 ปี
Dx : Suspicious for NHL
- case 2 มี reactive histiocyte มาก , cell ตัวใหญ่ cytoplasm สีชมพู , prominent nucleolus แต่เห็น abnormal lymphocyte ไม่ชัดเจน
- Case 4 พบ small lymphocyte มีลักษณะ indented(knot) , invade blood vv. และพบ large lymphocyte ปนกันอยู่
Case 3 :
Dx: reactive pleuritis
- granulomatous inflammation ที่ pleura
- pleura หนา มี subpleural fibrosis
- PAS , GMS : not found organism
- คิดถึง foreign body granuloma มากกว่า TB
Case 5 :
Dx : Adenosarcoma of endometrium
Cervic = - Uniform stromal cell, scant cytoplasm
- cell แทรกเป็นแถว อยู่ใน lymphatic vv.
- บาง area มีลักษณะเป็น gland
- cervical epithelium ปกติ
Myometrium = - benign gland ล้อมรอบด้วย stromal cell mass
- เห็น infiltrating margin เข้าไปใน myometrium
