เขียนอย่างไร ถูกไหมเนี่ย?
ด้วยพวกเราชาวพยาบาล(ตัวน้อยๆๆ)
ที่ทำงานหอผู้ป่วย 3ข รพ ศรีนครินทร์ก็จะมีน้องพยาบาลต่างชาติมาฝึกงานด้วย ด้วยเหตุนี้ต้องพาน้องๆเริ่มเรียนรู้ภาษาอังกฤษในการเขียน Nurse note เพื่อให้เขาได้เข้าใจเวลาที่จะสื่อสารในการทำงานร่วมกัน
(เป้าหมายก็คือน้องทุกคนที่อยากฝึกค่ะ)
แต่น้องพยาบาลใหม่ซิงๆ 3 เดือน ต้องฝึกเรียนรู้การอ่าน การค้นคว้าภาษาอังกฤษ มีเรื่องมาเล่าสู่กันฟัง
ลองอ่านดูนะคะแล้ววิจารณ์ด้วยค่ะ
การเขียนวิเคราะห์แต่ละปัญหายังไม่ได้ให้เขียนค่ะ
เขียนเฉพาะเวลาส่งเวรกับเวรต่อไป ว่าผู้ป่วยรายนี้เป็นอย่างไร
ในกรณีที่ปัญหาอิเลโตรลัยท์อิมบาลานท์ ฯลฯ แก้ไขอย่างไร ปัญหาเกิดจากอะไร (เฟสสองค่ะ , และต่อไปก็คือการฝึกรายงานแพทย์เป็นภาษาอังกฤษ.....
ที่สำคัญเรื่องการฝึกฟังให้น้องดูหนังเสียงในฟิล์ม และจากเวป youtube ค่ะ.....อิๆๆ ที่ทำน่ะหนุกหนานค่ะไม่กล้าหาญชาญชัยอะไรเลย อยากเปลี่ยนรสชาตกานทำงานมีหลายรูปแบบเฉยๆค่ะ.......เป็นช่วงการปรับตัวของน้อง.....)

Diagnosis:Esophageal leioma S/P esophagectomy with jejonostomy with dyaphagia
Pt is a good concious and oriented to times .E4V5M6 pupil 3+ reac to light both eye and strong at right arm and right leg.Not convulse.
Retain NG tube foe feed peptamen(1:1) 500 cc x 4 feeds with Motilium 1 x 3 oral ac .No neusia & vomiting and not retain content.
Wound at neck with uno drain seal hypafix dosen't has discharg ,content is pink color. Not complain wound.
His vital signs stable, afebrile.
He can void his self.He can stimulate ambulation.Observe clinical contineuos.

Diet: soft diet .No neusia & vomiting . He had no bowel sound movement today.
On condom transfer to urine bag . He can stimulate ambulation.Observ clinical contineuos.

Mr. H... medical history included: Alzheimer’s, senile dementia, hypertension, clinical depression, status post radical prostatectomy June 1996 secondary to prostate cancer, failure to thrive, gastro-entero reflux disease, and a history of skin cancer.
(The prescribed medications at the time of the incident were as follows)
: Aricept 10mg by mouth every night
Lipitor 10mg by mouth daily
Lasix 10mg by mouth daily
Potassium Chloride 750 mEq by mouth daily
Zoloft 10mg by mouth daily, Prilosec 20mg by mouth daily
Ativan 0.5mg by mouth every eight hours as needed for anxiety or agitation Tylenol (500mg=2tablets)1gm by mouth every eight hours by mouth as needed for pain or fever.
Mr. H....’s social history included: widower, three children, seven grandchildren, thirty-five year career in sales with the World Wide Import/Export Company of Khonkean garment. Mr. H.... lived alone for ten years in the Ban nong nok area until he was no longer able to care for himself. At which time, his oldest son, R..... H..... Jr., moved Mr. H.....to Carol’s Crossing with his family.
1 July, 2009 09 .43: Resident showered after breakfast with minimal assistance from staff. Weekly skin assessment done. Skin warm, dry and intact with poor turgor. Mucous membranes moist and pink. Quarter sized red area noted on left ischial tubersosity. Site blanches upon palpation. Moisture barrier applied to site and buttocks. Will continue to monitor. Medications taken at breakfast without difficulty. At this time the resident has left the unit for the prayer services held in the outside. We all know the alcoholic with severe liver disease, that comes in all yellow, big belly, low platelets, anemia, bleeding, low blood pressures, with metabolic encephalopathy, pooping all over the bed from the lactulose you are giving every four hours. They are so sick, it is not so difficult to give them some sedatives, get their bellies drained with paracentisis, transfuse some FFP, and maybe get them better enough to go home for a while longer.

the nurse trying to give IV potassium infusion plus oral replacement. Of course, the IV potassium kept burning, infiltrating etc, exacerbating the agitation(He wanted to leave).
the potassium was rechecked, it was still low and the fearful attending would not discharge the patient naturally. So what does the nurse do? She calls the doctor and gets orders to put the patient in restraints!(ใช่ค่ะผูกยึดไว้ก่อนเล้ยเดี๋ยวตกเตียง)
