บทคัดย่อ
การพัฒนาระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงาน เป็นการให้บริการด้านอาชีวอนามัยและความปลอดภัย ที่สามารถช่วยให้ผู้ป่วยสามารถกลับเข้าทำงานได้อย่างเหมาะสม มีความปลอดภัย
การติดตามหลังการประเมินผู้ป่วยก่อนกลับเข้าทำงานแบบต่อเนื่องนี้ มีวัตถุประสงค์เพื่อพรรณนาสถานการณ์ของผลลัพธ์การประเมินผู้ป่วยก่อนกลับเข้าทำงานในผู้ป่วยคนไทย และคนต่างด้าว เพื่อประเมินผลการติดตามการกลับเข้าทำงานของผู้ป่วยแบบต่อเนื่อง โดยศึกษาผลการติดตามผู้ป่วยหลังประเมินก่อนกลับเข้าทำงาน ในผู้ป่วยจำนวน 116 ราย โดยเลือกกลุ่มตัวอย่างจากแผนกที่มีผู้ป่วยคนไทยหรือต่างด้าว ที่มีแนวโน้มต้องกลับเข้าทำงาน ได้รับการประเมินก่อนกลับเข้าทำงานโดยแพทย์ พร้อมมีการติดตามผลการประเมินแบบต่อเนื่อง โดยเก็บข้อมูลตั้งแต่เดือนพฤศจิกายน 2557 ถึง เดือนพฤษภาคม 2558
เครื่องมือที่ใช้ประกอบด้วยแบบสอบถาม แบบบันทึกการเยี่ยมประเมินผู้ป่วยก่อนกลับเข้าทำงาน แบบประเมินก่อนกลับเข้าทำงาน แบบประเมินความพึงพอใจ แบบติดตามผลหลังการประเมินผู้ป่วยก่อนกลับเข้าทำงาน การสัมภาษณ์ สถิติที่ใช้ คือ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน
ผลการศึกษาพบว่าคนไทยสามารถเข้าถึงระบบการประเมินผู้ป่วยก่อนกลับเข้างานได้มากกว่าคนต่างด้าว โดยผู้ป่วยคนไทยที่ได้รับการประเมินผู้ป่วยก่อนกลับเข้าทำงาน มีจำนวน 77 คน(ร้อยละ 66.4) ผู้ป่วยต่างด้าวที่ได้รับการประเมินผู้ป่วยก่อนกลับเข้าทำงาน มีจำนวน 39 คน(ร้อยละ 33.6) ผู้ป่วยทั้งหมดที่ได้รับการประเมินก่อนกลับเข้าทำงาน ส่วนใหญ่เกินกว่าครึ่ง อยู่ช่วงอายุ 35 ปีขึ้นไป ส่วนใหญ่เป็นเพศชายมากกว่าเพศหญิง สามารถกลับเข้าทำงานเดิมได้ 60 ราย(ร้อยละ 51.1) ไม่สามารถกลับเข้าทำงาน 36 ราย(ร้อยละ 31) ปรับลักษณะงาน 18 ราย (15.5) ส่งฟื้นฟู 2 ราย (ร้อยละ 1.7) หลังพัฒนารูปแบบการประเมิน พบว่าหน่วยงานที่เข้าร่วมโครงการทั้ง 5 หน่วยงาน มีความพึงพอใจ คิดเป็นร้อยละ 84.44 เมื่อมีการติดตามหลังการประเมินผู้ป่วยก่อนกลับเข้าทำงานอย่างต่อเนื่องเมื่ออกจากโรงพยาบาล พบว่าผู้ป่วยที่สามารถกลับเข้าทำงานได้ มีจำนวน 60 คน (ร้อยละ 51.7) เมื่อให้คำแนะนำปรึกษา มีการติดตามดูแลอย่างต่อเนื่อง พบว่าสามารถกลับเข้าทำงานเพิ่มมากขึ้น 103 คน (ร้อยละ 88.88) เพิ่มขึ้น 1.72 เท่า จึงทำให้ทีมงานทราบว่าการพัฒนาระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงาน(return to work) ที่สำคัญต้องมีการพัฒนาระบบการติดตามประเมินผู้ป่วยกลับเข้าทำงานอย่างต่อเนื่อง พร้อมพัฒนาการส่งประเมินผู้ป่วยก่อนกลับเข้าทำงานของสถานประกอบการ กับโรงพยาบาลภาครัฐและภาคเอกชน ให้มีความรู้ ความเข้าใจในเรื่องระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงาน จึงจะทำให้ระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงานมีประสิทธิภาพ สามารถดูแลผู้ป่วยได้อย่างมีมาตรฐาน และมีความปลอดภัย
ดังนั้นจึงควรมีการร่วมมือกันพัฒนาระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงานระหว่างทีมสหวิชาชีพ และเครือข่ายภาครัฐภาคเอกชนแบบบูรณาการ ให้สามารถนำรูปแบบไปใช้ดูแลสุขภาพได้ทุกหน่วยงาน ทุกเชื้อชาติ อย่างมีมาตรฐาน มีความปลอดภัยแบบสากล ที่เป็นรูปธรรม เกิดการเชื่อมโยงระหว่างเครือข่ายพัฒนาระบบการประเมินผู้ป่วยก่อนกลับเข้าทำงานในระดับจังหวัด ระดับเขต ระดับประเทศ ที่สามารถดูแลสุขภาพ เพื่อนำสู่การมีสุขภาพที่ดี มีปลอดภัยในการทำงานแบบยั่งยืน
