I see in much smaller scales some like issues of seeing ‘the patient’ when a doctor is attacking her/his disease or ‘the students’ when ushering them through a course or ‘the farmers’ where an irrigation water dam is purposed. More often than not a passage raising improvement in some aspects to the patient/students/farmers/… will be included in the aims or goals. But there are NO REVIEW afterwards if that and other goals (apart from –structural/physical– “building”) are achieved.

We need ‘truly’ independent auditing committees (call them ‘overseers or reviewing boards’ or whatever names) on ‘budgetary’ projects. And we need a law that opens government document (including review reports) to public after 20 years from ‘completion date’ –to allow detailed public scrutity and academic study for improvement.
