I think "Rachel dived back in to rescue the struggling boy." here should be understood as "dived back in" (dived in again) and "to rescue".

If we said "Rachel dived back into rescueing the struggling boy.", grammatically we can say that but we could mean "Rachel was eager to take action in rescuing... the boy." Very subtle differences, eh? ;-)
