Yes, it is sad. Last month we opted to eat in clean looking foodshops rather to try the local varieties. Korat has one ก๋วยเตี้ยวสุโขทัย shop in town that was free of grease or grime on the table surface nor the bowl nor spoons -- perhaps it was recently opened. Pimai has one sidecart vendor selling pineapples, mangoes and guavas with the glass panes sparkle and more ice than water in the glass bins. (We bought pineapples -- they were sweet.)
We have been through more than 50 towns in the country and we did not buy in many towns. PTT deserves a credit for clean toilet. Most hotels we stayed have (our) better rating on cleanliness than when we toured last time.
