This conference supporting the studying of medical students, residenceis, health care provider staffs

We brought 1 case for learning that day .. with advance care plan and goad setting. This case was brought to the grand round both hematology grand round and palliative care grand round many times. He is a teenager, orphan, parents passed away while he was 5. His brother working as a worker in Bangkok. He is living with his uncle and aunty who have to taking care of thier children, 2 girls too. They are  the farmer. the problem of this case are as follow:

1. Not very well for his compliace: we had done family meeting, talking with the child and set goal together..

2. He is a teenager, need much attention while in hospital, severe joint pain for 2 months

3. And his disease relapse (ALL high risk third relapse)

4. financial problem

5. Psychological problem; low threshold of pain

We had discussed so many issues, we learn more from this case. After this grand round our team had discussed with his aunty and him..They would like to get chemo therapy again. So they (aunty and patient) promised to taking care of patient to increase the good compliance.

What do you think for long term care plan in this case?
