Effectiveness of implementing evidence-based practice for Traumatic patients at the Accident and Emergency Department (AED) of Srinagarind University Hospital, Khon Kaen, Thailand

ABSTRACT

Problem

In order to save trauma patients, a rapid and effective response is needed.  If the trauma nursing team is inadequately skilled, indecision would likely delay response time and/or complications may result from incorrect decisions.

Evidence

Search terms (fluid resuscitation, hypoxia, shock, suctioning, trauma, trauma nursing, and traumatic wound). We found eight relevant studies. The level of evidence A, B, and C were 1, 5, and 2, respectively.

Strategy

            We improved our nursing practice guideline (CNPG) for traumatic patients in our hospital based on the evidence. CNPG consisted of 4 sections (triage, primary survey, secondary survey, and pre discharge process).

Practice

Our CNPG was tested with traumatic patient at AED. Trauma Life Support and psychological were given.

Evaluation

            This study aimed to evaluated nurse’s action after we implement our CNPG practice in AED. We included all patients who received nurse service at AED. We recorded patient characteristic and nurse demographic data. We also recorded the completeness of nurse action in each section of CNPG by using peer checklist. We also recorded patient satisfaction on our service (0-10 point scale). Then, the data were statistically analyzed through the frequency distribution, percentage, mean, standard deviation as appropriated.

Results 

            We included 81 patients into studies. Trauma severity level of patients I, II, III were 6.2%, 4.9%, and 88.9%, respectively. The result showed that 90.5% of process nursing intervention met CNPG. The highest score for triage process (100%) followed by primary survey and secondary survey were middle score (97.94% and 93.62%) and the lower score for pre-discharge process (70.29%). Mean (SD) of patient satisfaction was 9.0 (1.1).

Recommendations

CNPG was important to improve nursing care quality. Implementation the CNPG into real situation did not simply. Emergency nursing management (triage, primary and secondary survey) in our AED was satisfaction. However, pre-discharge process was required improvement in our study. We suggested CNPG evaluation in real situation was important to improve nursing care in AED.

Lessons Learned

            Evidence-based practice for trauma care helps to coordinate and supervise nurses at the AED, to instill quality TLS in nursing team and to save patients.

สิ่งที่ได้เรียนรู้ในงานวิจัยนี้ คือ เรื่อง ทีมงาน เพราะการช่วยชีวิตผู้บาดเจ็บ เป็นเรื่องละเอียดอ่อน โดยงานวิจัยนี้ เราสร้างเครื่องมือ คือ CNPG เพื่อช่วยให้คนทำงานง่ายขึ้น เพราะเราพูดและทำงานในภาษาเดียวกัน และเพื่อพัฒนาสมรรถนะของผู้ปฏิบัติ ดังนั้นทีมงานผ่านการฝึกทั้งด้านความรู้และทักษะปฏิบัติ

เราจึงมั่นใจในคุณภาพการช่วยชีวิตของผู้บาดเจ็บ ค่ะ
