เม.ย. ๕๒ ผมไปเรียนรู้เรื่องการปฏิรูประบบสุขภาพของประเทศต่างๆ
ในโลก การทำความเข้าใจเรื่องระบบ
เรื่องการเปลี่ยนแปลงพัฒนาระบบนี้ ผมชอบ
ชอบแบบคลั่งใคล้ อยากรู้ อยากเรียน
และสนุกที่จะเรียน เพื่อเอามาทำหน้าที่คนแก่ ที่ยังมีคุณค่า
ผู้เชิญคือ

พิทยรังสฤษฏ์ แห่งสำนักพัฒนานโยบายสุขภาพระหว่างประเทศ (IHPP) เพื่อขอให้รับฟัง
และเสนอแนะต่อร่างผลงานวิจัยชื่อ
“องค์กรและการจัดการเชิงระบบในการขับเคลื่อนระบบสุขภาพอย่างมีส่วนร่วมในต่างประเทศ”
เอาเข้าจริง
กลายเป็นว่า ผมไปเรียนรู้วิชา stewardship
หมอศิริวรรณ สรุปภาพใหญ่ที่สุดของผลวิจัยว่า สช. ทำหน้าที่
stewardship ต่อการปฏิรูประบบสุขภาพ ซึ่งมีความหมายว่า
ทำหน้าที่เชิงรับใช้ ให้การเกื้อหนุน ให้คนอื่น/หน่วยงานอื่น
เป็นพระเอก/นางเอก ในการปฏิรูประบบสุขภาพ
ช่วยให้งานของคนอื่น/หน่วยงานอื่นประสบความสำเร็จดีเยี่ยม
แล้วมีผลต่อระบบสุขภาพของประเทศ
ทำให้ระบบสุขภาพของประเทศยกระดับขึ้น
ทีมวิจัยให้นิยาม ๓ แบบ ต่อ stewardship ดังนี้
• Stewardship in health is concerned
with exerting its influence on the other three ‘levers’ (financing,
resource generation, and service provision) so that these 3
functions can operate effectively. These responsibilities usually
falls under the jurisdiction of the Ministry of Health.
• Stewardship of health refers to
the strategic management role and is concerned with setting policy
directions for the development of health system according to the
social, demographic and economic environment. Furthermore, it is
also responsible for the development and maintenance of the
overarching organizational framework which deals with allocation of
functional responsibilities to certain units within the health
system.
• Stewardship for health involves
with activities which are beyond the health system which can have
an impact, either directly or indirectly on the main determinant of
health. In other words, this kind of stewardship looks to influence
and control the surrounding environment which the health system is
in.
ผมให้ความเห็นต่อทีมวิจัยว่า หากจะยึดหลักทำหน้าที่แบบ stewardship
ต่อการปฏิรูประบบสุขภาพ สช. ต้องสร้าง corporate culture,
ทักษะของเจ้าหน้าที่, และความสัมพันธ์กับพันธมิตรหรือผู้มีส่วนได้เสีย
ในลักษณะที่สอดคล้องกับแนวคิด/หลักการนี้
และต้องตีความหรือให้ความหมายของการทำหน้าที่ให้กว้างที่สุด
คือครอบคลุมความหมายทั้ง ๓ ข้อข้างบน
ถ้าจะให้ผมอธิบายคำ stewardship สั้นที่สุด ผมขอใช้คำว่า
นำจากข้างหลัง หรือนำแบบหนุน

ที่จะทำหน้าที่แบบนี้ได้ ต้องมีโลกทัศน์แบบ
