พัฒนาคุณภาพการบริการในร้านยา

พัฒนาร้านยา

วันนี้วันที่ 5 พ.ย. 2550 ผู้แทนจากร้านยาจังหวัด เจ้าหน้าที่กลุ่มงาน คบ.สสจ.ขอนแก่น และอาจารย์จากคณะเภสัชฯ  มข. ร่วมประชุมหารือแนวทางการพัฒนาคุณภาพร้านยาโดยเน้นที่ร้านขายยาแผนปัจจุบัน (ขย.1)

สิ่งที่เภสัชกรประจำร้านยาควรทราบคือ ทิศทางการพัฒนาในพื้นที่ จ.ขอนแก่นจะมุ่งเน้นที่ Metabolic syndrome prevention เป็นเรื่องที่บทบาทหน้าที่ใน้ร้านยาช่วยเสริมภาครัฐ และแนวโน้มของโรคจาก Metabolic syndrome; MS นับวันจะมากขึ้น ซึ่งมักเกิดขึ้นจากพฤติกรรมผู้บริโภค คือ 3 อ.  เริ่มตั้งแต่การดูแลตนเองในการออกกำลังกาย สูดรับอากาศที่บริสุทธิ์ และการบริโภคอาหารโดยเฉพาะอาหารที่มีรสหวาน-มัน-เค็มทั้งหลาย

ส่วนเกณฑ์การวินิจฉัยของภาวะ MS (ไม่ใช่ MicroSoft นะครับ) ที่เหมาะสมเป็นของ IDF (ตามตารางข้างล่าง) ที่ได้พัฒนาจากเกณฑ์ของ WHO และ NCEP Criteria ซึ่งอาจารย์วิวรรธน์ อัครวิเชียรได้นำเสนอในการประชุมครั้งนี้ซึ่งอ้างอิงจากเอกสารของ ศ.นพ.ปิยทัศน์ ทัศนาวิวัฒน์ ซึ่งเขียนไว้ในวารสารอายุศาสตร์อีสาน ปีที่ 6 ฉบับที่ 3 เดือนกรกฎาคม-กันยายน 2550 ในหัวข้อเรื่อง “Metabolic syndrome prevention”

 Waist circumference*-ethnicity specific 

 Plus any two:

 Raised triglycerides  > 150 mg./dL. 

Specific treatment for this lipid abnormality

 Reduced HDL-cholesterol > 40 mg./dL. in men, > 50 mg./dL. in women

Specific treatment for this lipid abnormality 

Raised blood pressure 

 Systolic > 130 mm. Hg. 

Diastolic > 85 mm. Hg. 

Treatment of previously diagnosed hypertension 

Raised fasting plasma glucose?

Fasting plasma glucose > 100 mg./dL. 

Previously diagnosed type 2 diabetes

If above 100 mg./dL, oral glucose tolerance test is strongly recommended, but is not necessary to define presence of syndrome 

* If body-mass index is over 30 kg./m² , central obesity can be assumed and waist circumference does not need to be measured. In clinical practice, impaired glucose tolerance is also acceptable, but al report of prevalence of metabolic syndrome should use only fasting plasma glucose and presure of previously diagnosed diabetes to define hyperglycaemia. Prevalence also incorperating 2-h glucose results can be added as supplementary findings.
