            สำหรับรายสุดท้ายนี้ ต้องเรียก ว่า โคตรหิน

เป็นคนไข้ เบาหวาน ที่เคยมี Osteomyelitis ของ Calcaneous กินจน Bone ทื่ insertion ของ Achilles tendon หายไป คนไข้ไม่สามารถ ทำ Ankle plantar flexion ได้เลย  กล่าวคือ คนไข้ไม่ สามารถ Transfer weight ไปยัง Fore foot ใน Stance phase ได้เลย

                การรักษา โดย Total contact cast ได้ผลดี แต่การ ป้องกัน Recurrent ulcer นั้น ต้องควบคุม Ankle joint และBy pass weight จาก Patella ลงสู่ Double upright bar สู่ Stirup ที่ Attach กับ Mid sole ถ่ายลงสู่ Out sole –Ground ตามลำดับ ครับ
