100 "mcg per hr" Fentanyl  :about  x2 to 2.5 = 200-250  in mg per day of oral Morphine 

and

100 mg per day oral Morphine : about  /2 to 2.5 = 50-40 in mcg per hr of Fentanyl patch

Reasons
Fentanly 100 times potent : 100 mcg = 10 mg MO oral
24 hrs                                            = 240 mg MO oral  per day

Lower end for renal impairment
Higher end for opioid intolerant
