1. เหตุผลในการไปศึกษาดูงานการจัดการความปวดเรื้อรังในเด็ก (Complex pain) และการวิจัย และได้แลกเปลี่ยนประสบการณ์ความคิดเห็นและเพิ่มพูนความรู้เรื่องการดูแลผู้ป่วยเด็กที่มีความปวด รวมทั้งการดูผู้ป่วยเด็กระยะสุดท้ายกับทีมวิจัยและทีมสหสาขาวิชาชีพที่แคนาดาและอเมริกา และเข้าเรียน 1 สัปดาห์ในหลักสูตร Pediatric pain master class ที่จัดขึ้นเป็นครั้งที่ 3 สำหรับแพทย์และพยาบาลผู้ปฏิบัติการขั้นสูง (Physicians and Advance nursing practice) จัดโดย Children’s institute for pain and palliative care (CIPPC), Pain and palliative care program, และ Children Hospitals and clinics of Minnesota |

จากการเข้าเรียนThe pain master class 1สัปดาห์ กระบวนการเรียน ฟังบรรยาย ฝึกปฏิบัติ ดูงาน เป็นการจัดครั้งที่ 3 ที่ Marquette Hotel, Minneapolis, MN, USA เมื่อวันที่ 12-18 มิย. 2553 ที่ผ่านมา ได้ความรู้ แนวคิดเรื่องการจัดการความปวดในเด็ก โดยการจัดการประชุมครั้งนี้ จัดให้เฉพาะแพทย์ (physicians) และ advance nursing practice จำนวน 60 คน ผู้เรียนมาจากทั่งโลก อเมริกา แคนาดา ออสเตรเลีย นิวซีแลนด์ ไอแลนด์ ฟินด์แลนด์ ฝรั่งเศษ จีน อินเดีย คูเวต และประเทศไทย
Day 1 เริ่ม 7.30 น. ก่อนเรียนทุกคนแนะนำตัว
เราเรียนอยู่ชั้น 50 ค่ะ
Intended Audience เฉพาะ แพทย์ (Physician) และพยาบาล (APN)
This seminar offers state-of-the-art education in pain management for the
pediatric patient from a holistic and multidisciplinary perspective. The
program features a faculty of internationally recognized experts, who will
cover pharmacological, medical, psychosocial, and integrative therapies in the
management of children’s acute and complex/chronic pain. Pain assessment and
management of somatic, visceral, neuropathic, and psychosocial/spiritual pain,
as well as end-of-life management of pain and distressing symptoms will be
discussed in depth using lectures, workshops, and small-group approach.
The master class is primarily designed for physicians and advanced practice
nurses to develop their expertise in the field of pain management in a highly
interactive seminar format. Space is limited.

5. Discuss guidelines for implementing sedation to unconsciousness for intractable pain in pediatric palliative care
6. Identify ethical decision making frameworks and strategies for complex pain management needs in palliative care
7. Explore an interdisciplinary model for management of complex/chronic pain (migraines and tension headaches, functional abdominal pain, CRPS, musculoskeletal pain, conversion disorders, etc.)
8. Identify implications of diverse family system dynamics, and psychosocial considerations in care planning and implementation
9. Identify integrative and complementary strategies for pain and symptom management, including massage, biofeedback, hypnosis, aromatherapy, etc.
10. Identify rational approaches to post-operative pain control
11. Describe best practice approaches to neuroirritability
12. Evaluate comprehensive pain assessment strategies for children with diverse abilities and levels of consciousness
13. Identify psycho-social and spiritual dimensions in pain and
symptom management
14. Learn innovative approaches to procedural pain, including nitrous oxide administration
15. Learn how to address opioid-induced hyperalgesia
16. Discuss interdisciplinary strategies for effective pain and symptom management in diverse situations
17. Identify the broad continuum of pain and symptom management challenges and strategies in pediatric care
18. Learn strategies for developing an interdisciplinary team approach to pain and symptom management
19. Evaluate strategies for networking with colleagues in this growing specialty field

ดีใจมากที่เจอ Jody อีกครั้ง Jody เป็นพยาบาล Program pain palliative care co-ordinator ของ Pain and Palliative care ที่นี่ เมื่อปี 2003 ฉันมาดูงานด้านการดูแลผู้ป่วยเด็กระยะสุดท้ายที่นี่ เป็นเวลา 4 เดือน Jody คือ Host family ที่ดีมาก ทั้งเพื่อนๆ และครอบครัวค่ะ
นอกจากเรียน (หนัก) มีทัวร์โรงพยาบาลเด็กด้วยค่ะ
เด็กที่รักษาตั้งแต่เล็กๆ ตอนนี้โต มีคุณภาพชีตที่ดี
โรงพยาบาล สะอาด สวยงาม พยาบาลไม่ใส่ uniform จะใช้ป้าย ID แยกตำแหน่ง
คุณพ่อคุณแม่ พาลูกที่นอนรักษา โดยมีรถลากน่ารัก ออกไปชมบรรยากาศข้างนอก
ห้องคนไข้ น่ารักมาก อยู่ทั้งครอบครัว เหมือนโรงแรม
เพื่อนๆ ใน Class ส่วนใหญ่เป็นวิสัญญีแพทย์ แพทย์เด็กมะเร็ง ฯลฯ ตอนนี้เรารอรถมารับ ตอนไปทัวร์ Childrens Hospitals and Clinic, MN จากไทย มีวิสัญญีแพทย์อาจารย์อ้อวิมลรัตน์ ศรีราช, อาจารย์หน่อย เดือนเพ็ญ ห่อรัตนาเรือง และฉัน
