The Quality and Healthy House Model


The new model fo hospital improvement of quality care developed by action research in Bantak hospital, Bantak District, Thailand.

 I started my work in Bantak hospital 10 years ago. In that time, Bantak hospital was 30 beds hospital. By National policy, it could'nt expand or growth becaus of the Economic crisis, so I and my staff tried to develop good quality of care with insufficient routine budgeting. And the new model was developed.

            “The Quality and Healthy House Model” was developed by BanTak Hospital for being the strategic model of how to improve the service quality in order to promote good health. This model is developed based on 5S activities that can be compared to the basement of a house. Organisation Development activities and Excellence in Service behavior could be compared as a house’s wall since health services must be done in teamwork and everybody in the team need to behave well. Total Quality Management, Suggestion system, CQI (Continuous Quality Improvement) and Total Healthy Management are compared to the house’s body. Moreover, Plan-Do-Check-Act could be compared to 4 gateways of the house, while Hospital Accreditation is like the first level of the roof built for protecting the house from sunlight and rain. Health Promoting Hospital standard is the gable built for warmth and happiness. The bell on the top of the gable represents the three foremost goals: Healthy citizen, Happy employees and Survival organisation. As a whole, the main purpose is the concept of Healthy Thailand Campaign. 

         10 ways of integration contributing to Healthy Thailand are as following: 

1.      Starting from the basement: Applying 5S activities

2.      Constructing a cozy house: Creating the workplace that motivates the staffs and promotes physical and mental happiness.

3.      Knowledgeable leader: The leader has to expert in four areas of administration: planning, organizing, leading and controlling. Additionally, he/she has to be capable in strategic planning and result - based management.

4.      Shared Vision:  Sharing goals. Generating competency and commitment by supporting continuously learning.5.      Unity is power:  Encouraging the staffs to work in team.

6.      Team Learning: Three modules of teamwork have to be adapted: “leading” team, “working” team and “supporting” team.

7.      Focusing on standards: Emphasising the importance of HA standards that focus on the benefits of people, the importance of teamwork and the value of individuality.

8.      Starting to practice: There are three steps that have to be done: 1) Self-reflecting, 2) Accrediting and 3) CQI (Continuous Quality Improvement).These three steps are connected to each other.

9.      Evaluating: Setting internal and external assessment to find out any faults resulting in development.

10. Straight to the goal: Three balancing goals: Healthy citizen, Happy staffs and Survival Organisation have to be emphasized. 

The Quality and Healthy House Model is shown in figure.  

5S activities are considered as the efficient tools for quality assurance. 5S is developed in Japan and stands for :

S1 : SEIRI : (Clearing Up)

S2 : SEITON : (Organizing)

S3 : SEISO : (Cleaning)

S4 : SEIKETSU : (Standardizing)

S5 : SHITSUKE : ( Training & Discipline)
