How Oxycodone different from oxycontin

Interesting : Oxycodone is not pure Oxycodone!

Chronic pain:

Titration with escalating concept as Acute pain

Maintanance dose 2/3 of  24 hrs titrated needed dose
Bolus/breakthrough 10% of 24 hrs titrated needed dose

Escalating opioid.

Opioid naive with mod-sev pain -> giving Morphine 2- 5 mg IV

Re-evaluate 15 min ( - this is the peak effect, however, pt should show some relieve at 5 min after prescribeed)

Escalating dose if not feel better

mild to mod     : 25-50%
severe             : 50-100 %
Percentage reduce for elder /poor renal or liver function.

eg. 5 mg IV --> pt still show crying, highly pain : 5+5 = 10

tip converting fentanyl patch and Morphine oral

100 "mcg per hr" Fentanyl  :about  x2 to 2.5 = 200-250  in mg per day of oral Morphine 

and

100 mg per day oral Morphine : about  /2 to 2.5 = 50-40 in mcg per hr of Fentanyl patch

Reasons
Fentanly 100 times potent : 100 mcg = 10 mg MO oral
24 hrs                                            = 240 mg MO oral  per day

Lower end for renal impairment
Higher end for opioid intolerant
