21พค. 2553 Palliative grand round เรานำเคส เด็กชาย 4 ขวบ ป่วยด้วยมะเร็งเม็ดเลือดขาว ALL high risk ตั้งแต่อายุได้ 2 ปี มา conference และวางแผนดูแลต่อเนื่อง รวมทั้งการนำหลายๆ ประเด็นมาพูดคุยเรียนรู้ ผู้เข้าร่วมกิจกรรม ประกอบด้วย อาจารย์แพทย์ Hematology 2 ท่าน พยาบาล ครู นักสังคม นักศึกษาแพทย์ และแพทย์ประจำบ้าน ทำให้นักศึกษามีโอกาสได้เรียนรู้ว่าในแต่ละประเด็น จะ approach และวางแผนดูแลต่อเนื่องร่วมกับครอบครัวเพื่อคุณภาพชีวิตที่ดี เพราะการดูแลผู้ป่วยเด็กระยะสุดท้าย ต้องครอบคลุม กาย จิต อารณ์ สังคม และจิตวิญญาณ โดยทำงานร่วมกันเป็นทีมสหสาขาวิชาชีพ
โดยก่อนนำเข้าสู่ case conference ท่าน รศ. นพ. สุรพล ได้เปิดประเด็นเพื่อให้นักศึกษาแพทย์ได้แลกเปลี่ยน เช่น palliative care คือ อะไร ทำไมจึงมีชั่วโมงการเรียนแลกเปลี่ยนร่วมกันแบบนี้

- They are worry about their child suffering ..last time staying one month in hospital.. father said
- Philladelphia positive.. CNS relapse that means very bad outcome
- OUTCOME ..poor.. parents well knew about..but they would like him living comfortably
Dr Surapon mention about …bad news to amputate the leg of patient something..How to approach..
- Palliation should look after the best …Counseling the final decision ..depending on family-patient decide
- Enough information by appropriate counseling would be the best to do
- Back to this case จักรพรรดิ์ total informed to parents by Dr Surapon..chemo IT for decrease IICP for patient quality of life.. What do you think?
- Parents well understanding this current situation … but so very sad to hear…very frustration…then do not talk more..
- Mother can accept palliative care..would like to see her child comfortably…crying… Resident said
- What different total and not total palliation..? Curative and palliative should come at first together since patient was diagnosed
- What is your priority to manage palliation condition in this case..?
- Palliative care is an intensive care…active total care.. physical.. relief symptom distress ..psychosocial and spiritual care..
- Concern patient’s quality of life.. providing care /treatment.. comfort/support care as much as we can.. by multidisciplinary care team approach.. then..
- Teacher at child life program.. social worker..
- It is very difficult time for family .coping with dying condition of the child
- Concern family’s relationship father – mother relationship ..what is going now..?
- At the end.. should focus more about psychosocial and spiritual support follow as any context.. family belief.. etc
- Chronic case increase in Thailand each year.. so palliative care would very much concerned in Thailand
